Become a new patient

We are not accepting new patients for Functional Medicine care. We are currently accepting patients for laser and/or PEMF appointments only.

When we receive this submission, we will call or email you to answer any questions and schedule your first appointment. We will email you a New Patient Form that you can print and complete. If you do not have printing capability, we can mail you the forms, if you provide us your mail address.

The forms can take up to an hour to complete. Please complete the New Patient Form before your first appointment and bring it with you. If not completed, your appointment will be rescheduled.

We do not accept insurance and do not participate in Medicare.
